Payment Options
Option 1
Paid in Full
Pay $2,190 in full, by cash or check, at the initial appointment and receive a 5% discount.
Option 2
2 Part Payment
Pay $1,095 at the initial appointment and $1,095 at the delivery of the appliance.
This can be paid with cash, check, or credit card (MasterCard or Visa).
Option 3
12 month payment plan
Pay $600.00 at the initial appointment. Sign up for automatic credit card charge of $125 for the next 12 months.
Option 4
Care Credit Contract
Establish a payment plan contract with Care Credit. Care Credit will pay the doctor for full service and the patient will then be responsible for paying Care Credit instead of the doctor.
This treatment fee includes:
Adjustments for 3 months post-delivery
2 years re-evaluation 6 mos. intervals (yearly thereafter)
Repair/reline up to 3 months post-delivery
Hardware warranty varies with brand/type of appliance
The treatment fee excluded follow-up sleep studies and physician visits.

We will facilitate health insurance reimbursement claims to your health insurance provider however coverage/reimbursement is up to the discretion of your insurer.
